Cook County Health Survey on Child Vaccinations

Cook County Health is gearing up for approval of the vaccine for children 12 years and older.  They are hoping the approval will come next week.  In an effort to be proactive, they have asked if we would reach out to you to send the survey below to parents and guardians within your school district. 

The survey does not request any personal information.  It just asks if they have kids in certain age ranges, and if they would like to have their kids vaccinated.
